Magnesium sulfate acts as a physiological calcium channel blocker. It inhibits calcium influx into presynaptic nerve terminals, reducing acetylcholine release at the neuromuscular junction and decreasing muscle contraction. It also antagonizes NMDA receptors and stabilizes neuronal membranes.
Meropenem is a carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
IV: Loading dose 4-6 g over 20-30 minutes, followed by maintenance infusion 1-2 g/hour for seizure prophylaxis in severe preeclampsia/eclampsia. IM: 4-8 g deep IM initially, then 4 g every 4 hours as needed.
