This leads to venous and arterial dilation, reducing preload and afterload, thereby decreasing myocardial oxygen demand.
Nitroglycerin is a prodrug that is converted to nitric oxide (NO) in vascular smooth muscle, activating guanylyl cyclase, increasing cGMP, leading to vasodilation primarily in veins and arteries.
20 mg orally twice daily, 7 hours apart (e.g., 8 AM and 3 PM) to provide a nitrate-free interval.
