Mechanism of Action
NALBUPHINE

Mixed opioid agonist-antagonist; agonist at κ-opioid receptors and antagonist/partial agonist at μ-opioid receptors.

DIASTAT

Diazepam enhances the effect of gamma-aminobutyric acid (GABA) at GABA-A receptors, increasing chloride ion conductance and neuronal hyperpolarization, leading to anxiolytic, sedative, muscle relaxant, and anticonvulsant effects.

DIASTAT (diazepam) is classified as Pregnancy Category D. First trimester: Increased risk of congenital malformations, particularly cleft lip and palate, when used during the first trimester. Second and third trimesters: Chronic use may lead to fetal dependence and withdrawal symptoms postnatally; risk of floppy infant syndrome (hypotonia, lethargy, sucking difficulties) when administered near term.

Lactation Summary

Excreted in human milk in low concentrations (M/P ratio ~0.6). Relative infant dose estimated 0.5-1% of maternal weight-adjusted dose. Monitor infant for sedation and poor feeding. American Academy of Pediatrics considers compatible with breastfeeding with caution.

NALBUPHINE

Nalbuphine is a mixed agonist-antagonist opioid with a ceiling effect for respiratory depression, making it safer than pure agonists. It can precipitate withdrawal in opioid-dependent patients. Monitor for sedation and hypotension. Reversal with naloxone may be less effective. Use with caution in hepatic impairment. Not recommended for chronic pain due to psychotomimetic effects.

DIASTAT

DIASTAT (diazepam rectal gel) is a formulation for acute management of seizure clusters. Administer rectally; monitor for respiratory depression, especially with concomitant CNS depressants.
