Mechanism of Action
NALOXONE HYDROCHLORIDE AND PENTAZOCINE HYDROCHLORIDE

Pentazocine is a mixed agonist-antagonist opioid that binds to mu-opioid receptors (partial agonist) and kappa-opioid receptors (agonist), producing analgesia. Naloxone is a pure opioid antagonist that competitively blocks mu, kappa, and delta receptors; when administered orally, naloxone undergoes extensive first-pass metabolism, reducing systemic absorption and primarily blocking the effects of pentazocine if the combination is misused parenterally.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin binds to antithrombin III (ATIII), accelerating its inhibition of coagulation factors Xa and IIa (thrombin). Its anti-factor Xa to anti-factor IIa activity ratio is approximately 3.6:1.

Standard Dosing
NALOXONE HYDROCHLORIDE AND PENTAZOCINE HYDROCHLORIDE

Oral: One tablet (naloxone 0.5 mg / pentazocine 50 mg) every 3-4 hours as needed for pain; maximum 12 tablets daily.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

1 mg/kg subcutaneously every 12 hours or 1.5 mg/kg subcutaneously once daily. For prophylaxis: 40 mg subcutaneously once daily or 30 mg subcutaneously every 12 hours.

Metabolism
NALOXONE HYDROCHLORIDE AND PENTAZOCINE HYDROCHLORIDE

Pentazocine is metabolized primarily by hepatic conjugation (glucuronidation) and oxidation via CYP2C19 and CYP2D6; naloxone is extensively metabolized by the liver, primarily via glucuronidation (UGT2B7).

Enoxaparin is primarily metabolized in the liver via desulfation and depolymerization, with some renal clearance. It does not rely on cytochrome P450 enzymes.

Excretion

Spinal/epidural hematomas may occur in patients receiving enoxaparin who are undergoing neuraxial anesthesia or spinal puncture, resulting in long-term or permanent paralysis. Risk is increased by use of indwelling epidural catheters, concomitant use of other anticoagulants, or history of spinal surgery/deformity. Monitor for signs of neurological impairment and manage emergently.

Second/Third trimester: Chronic use may cause fetal dependence; neonatal withdrawal syndrome reported. High doses near term may cause neonatal respiratory depression.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin does not cross the placenta and is considered low risk for teratogenicity. No increased risk of congenital anomalies has been reported in humans. First trimester: no known teratogenic effects. Second trimester: no known fetal harm. Third trimester: risk of maternal hemorrhage, which may indirectly affect fetus; use with caution.

Lactation Summary

NALOXONE HYDROCHLORIDE AND PENTAZOCINE HYDROCHLORIDE

Naloxone in this fixed-dose combination is included to deter opioid abuse by reversing euphoria. The pentazocine component is a mixed agonist-antagonist opioid; naloxone has poor oral bioavailability but becomes active parenterally, precipitating withdrawal in opioid-dependent individuals. Use with caution in patients with impaired renal or hepatic function. Monitor for respiratory depression, especially in opioid-naive patients, as pentazocine alone can cause respiratory depression.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin is a low molecular weight heparin (LMWH) preferred over unfractionated heparin for VTE prophylaxis due to predictable pharmacokinetics and no need for routine a PTT monitoring. Adjust dose for renal impairment (Cr Cl <30 m L/min). Protamine sulfate partially reverses (about 60%) its anticoagulant effect.
