Iodinated contrast agent that attenuates X-rays, providing vascular and tissue opacification by increasing the density of blood vessels and organs.
Barium sulfate acts as a radiopaque contrast agent. It has high atomic number (z=56) and density, which attenuates X-rays and provides positive contrast in the gastrointestinal tract. It is not absorbed systemically and coats the mucosal surface, allowing visualization of luminal anatomy and pathology.
Intravenous: 1-2 mL/kg (300 mg I/mL) for contrast-enhanced CT; intra-arterial: 5-80 mL per injection depending on procedure; maximum total dose 4 mL/kg.
125 mL orally once for upper GI studies; 250-500 mL orally once for small bowel follow-through. Not for IV use.

The terminal elimination half-life of iohexol in patients with normal renal function (creatinine clearance > 90 mL/min) is approximately 1.5 to 2 hours. In patients with renal impairment, the half-life is significantly prolonged (up to 30 hours or more in severe renal failure), necessitating dose adjustment and careful monitoring.

Omnipaque 300 (iohexol) is primarily eliminated unchanged by the kidneys via glomerular filtration. Billiary excretion is minimal, with less than 0.1% recovered in bile or feces.
Varibar (barium sulfate) is not absorbed from the GI tract; it is excreted unchanged in feces. 100% fecal elimination as unabsorbed barium sulfate.
