Orvaten is a purified form of tetrahydrobiopterin (BH4), a cofactor for aromatic amino acid hydroxylases including phenylalanine hydroxylase (PAH), tyrosine hydroxylase, and tryptophan hydroxylase. In patients with phenylketonuria (PKU), it enhances the activity of residual PAH, leading to increased metabolism of phenylalanine and reduced blood phenylalanine levels.
Vasodilator (arteriolar dilator) reducing afterload; also inhibits platelet aggregation through inhibition of phosphodiesterase III.
5 mg orally twice daily
Oral: 2.5-5 mg twice daily; maximum 20 mg daily.

Terminal half-life: 8-12 hours in healthy adults; prolonged to 20-30 hours in severe hepatic impairment necessitates dose adjustment.
Terminal elimination half-life is 3-4 hours in healthy individuals, prolonged to 7-15 hours in renal impairment and in elderly patients. Clinical context: dosing interval adjustment recommended for CrCl <30 mL/min.
Renal: 60% unchanged; Biliary/fecal: 30% as metabolites; 10% exhaled as CO2.
Pindac (pindolol) is eliminated predominantly via hepatic metabolism (60-65%) with renal excretion of unchanged drug (35-40%). Less than 1% is excreted in feces via biliary elimination.
