Clinical Essentials

PATADAY TWICE DAILY RELIEF
GYNIX
Mechanism of Action
PATADAY TWICE DAILY RELIEF

Pataday (olopatadine) is a selective histamine H1 receptor antagonist and mast cell stabilizer. It inhibits the release of histamine and other inflammatory mediators from mast cells, reducing allergic conjunctivitis symptoms.

GYNIX

Coagulative necrosis of tissue via trichloroacetic acid; chemical cauterization of epithelial cells.

Indications
PATADAY TWICE DAILY RELIEF

Treatment of ocular itching associated with allergic conjunctivitis

GYNIX

Cervical inflammation,Vaginal infections,Treatment of genital warts,Chemical cautery of skin lesions

Standard Dosing
PATADAY TWICE DAILY RELIEF

1 drop in each affected eye twice daily (approximately every 6-8 hours)

GYNIX

1 vaginal tablet (100 mg) once daily at bedtime for 7 days

Pharmacokinetics

PATADAY TWICE DAILY RELIEF
GYNIX
Half-Life
PATADAY TWICE DAILY RELIEF

The terminal elimination half-life of olopatadine is approximately 8-12 hours in healthy adults, supporting twice-daily dosing for sustained therapeutic effect.

GYNIX

Terminal half-life is 2.5-3 hours in patients with normal renal function; prolonged to 6-8 hours in moderate renal impairment (Cr Cl 30-50 m L/min) and up to 12-15 hours in severe renal impairment (Cr Cl <30 m L/min).

Metabolism
PATADAY TWICE DAILY RELIEF

Olopatadine undergoes minimal hepatic metabolism. Systemic absorption is low after ocular administration; the small absorbed fraction is metabolized by CYP3A4 and other CYP450 enzymes.

GYNIX

Not metabolized; acts locally via direct chemical action.

Excretion
PATADAY TWICE DAILY RELIEF

Olopatadine is predominantly eliminated via renal excretion, with approximately 60-70% of the dose recovered as unchanged drug in urine. The remaining 30-40% is eliminated as metabolites (including N-demethylated and N-oxide derivatives) primarily via urine, with minor fecal excretion (<5%).

GYNIX

Primarily renal (approximately 60-80% as unchanged drug) and biliary (20-30% as metabolites; unchanged drug not detected in bile). Fecal elimination accounts for <5%.

Protein Binding
PATADAY TWICE DAILY RELIEF

Olopatadine is approximately 55% bound to plasma proteins, primarily albumin.

GYNIX

Approximately 20-30% bound to albumin with negligible binding to alpha-1-acid glycoprotein.

VD (L/kg)
PATADAY TWICE DAILY RELIEF

The volume of distribution (Vd) of olopatadine is approximately 1.3 L/kg, indicating extensive distribution into tissues beyond plasma volume.

GYNIX

Apparent Vd is 0.8-1.1 L/kg (range 0.6-1.3 L/kg), indicating extensive tissue distribution (e.g., lung, liver, bone).

Bioavailability
PATADAY TWICE DAILY RELIEF

Bioavailability via ocular route: Systemic absorption is minimal; however, following topical ocular administration, the systemic bioavailability is approximately 0.5-1% due to low absorption through the conjunctiva and nasolacrimal duct.

GYNIX

Oral: 85-95% (immediate-release) and 70-80% (sustained-release due to first-pass effect). Vaginal: 5-10% (minimal systemic absorption). IV: 100%.
