PROLOPRIM vs SEPTRA GRAPE

Inhibits bacterial dihydrofolate reductase (DHFR), blocking the conversion of dihydrofolic acid to tetrahydrofolic acid, thereby inhibiting bacterial DNA, RNA, and protein synthesis.
Septra Grape (trimethoprim/sulfamethoxazole) inhibits bacterial folic acid synthesis via sequential blockade: sulfamethoxazole inhibits dihydropteroate synthase, and trimethoprim inhibits dihydrofolate reductase, leading to bactericidal activity.
100 mg orally twice daily or 200 mg orally once daily.
160 mg trimethoprim / 800 mg sulfamethoxazole (1 double-strength tablet) orally every 12 hours.

Terminal elimination half-life is 8-10 hours in normal renal function; prolonged (>20 hours) in significant renal impairment.
