RENOQUID vs SULFATRIM PEDIATRIC

RENOQUID is a combination of sulfamethoxazole, an intermediate-acting sulfonamide, and trimethoprim, a dihydrofolate reductase inhibitor. It inhibits sequential steps in bacterial folic acid synthesis: sulfamethoxazole inhibits dihydropteroate synthase, and trimethoprim inhibits dihydrofolate reductase, leading to bactericidal activity.
Sulfamethoxazole inhibits dihydropteroate synthase, blocking bacterial folic acid synthesis; trimethoprim inhibits dihydrofolate reductase, blocking reduction of dihydrofolate to tetrahydrofolate. Sequential blockade leads to bactericidal activity.
100 mg orally twice daily
Sulfatrim Pediatric suspension contains sulfamethoxazole 200 mg and trimethoprim 40 mg per 5 mL. For patients >40 kg, dose is 800 mg SMX/160 mg TMP orally every 12 hours for 10-14 days.

Terminal elimination half-life is 2.5 hours (range 2–3 hours) in patients with normal renal function. In renal impairment (CrCl <30 mL/min), half-life may extend to 8–12 hours.
Sulfamethoxazole: 9-11 hours; Trimethoprim: 8-10 hours; prolonged in renal impairment (e.g., CrCl <30 mL/min).
Renal excretion accounts for approximately 70% of elimination, with 30% excreted unchanged in urine. Biliary/fecal excretion accounts for 30%, primarily as metabolites.
Renal: 50-70% of total sulfamethoxazole (SMX) and 30-50% of total trimethoprim (TMP) are excreted unchanged in urine; the remainder as metabolites; biliary/fecal excretion is minimal.
