SEGLENTIS vs TRIAMCINOLONE

SEGLENTIS is a fixed-dose combination of the opioid oxycodone and the opioid antagonist naltrexone. Oxycodone acts as a mu-opioid receptor agonist, providing analgesia. Naltrexone is intended to reduce the abuse potential of oxycodone by blocking opioid receptors when the drug is tampered with (e.g., crushed or chewed), but is sequestered in the core of the tablet and not released when taken orally as directed.
Synthetic glucocorticoid with anti-inflammatory, immunosuppressive, and anti-allergic effects. Binds to glucocorticoid receptors, modulating gene expression to inhibit phospholipase A2, reduce prostaglandin and leukotriene synthesis, and suppress cytokine production.
Subcutaneous injection: 300 mg (1.5 mL) once weekly. Administer in combination with oral capecitabine.
Intramuscular: 40-80 mg as a single dose; Intra-articular: 5-40 mg depending on joint size; Topical: Apply thin layer 2-4 times daily; Oral: 4-48 mg/day in divided doses.

moderateThe terminal elimination half-life of celecoxib is approximately 11 hours; for tramadol, it is about 6 hours, and for its active M1 metabolite, about 7 hours. Clinically, this supports twice-daily dosing for Seglentis (two tablets BID).
The terminal elimination half-life of triamcinolone is approximately 2-5 hours (mean 3 hours) following intravenous administration. Clinically, this short half-life supports multiple daily dosing for systemic effects, but duration of action is longer due to receptor occupancy.
Seglentis (celecoxib and tramadol) is primarily excreted renally. Celecoxib is eliminated via hepatic metabolism (CYP2C9) with <3% excreted unchanged in urine; fecal excretion accounts for approximately 70% of an oral dose (as metabolites). Tramadol and its active metabolite (M1) are mainly excreted renally (about 90% of the dose, with 30% unchanged tramadol and 15% M1); the remainder is excreted fecally.
Triamcinolone is primarily metabolized hepatically; unchanged drug and metabolites are excreted renally. Approximately 25-30% of a dose is excreted in urine as unchanged triamcinolone, with the remainder as metabolites. Fecal excretion accounts for less than 10%.
