SULFAMETHOXAZOLE AND TRIMETHOPRIM DOUBLE STRENGTH vs XIFAXAN

Sulfamethoxazole inhibits bacterial dihydropteroate synthase, blocking folic acid synthesis. Trimethoprim inhibits bacterial dihydrofolate reductase, blocking reduction of dihydrofolate to tetrahydrofolate. Sequential blockade produces bactericidal effect.
Rifaximin is a non-systemic, gut-selective antibiotic that inhibits bacterial RNA synthesis by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase, thereby reducing bacterial overgrowth and altering gut microbiota composition.
One double-strength tablet (160 mg trimethoprim/800 mg sulfamethoxazole) orally every 12 hours.

Sulfamethoxazole: 9-11 hours; trimethoprim: 8-11 hours. In severe renal impairment (CrCl <15 mL/min), half-life prolongs significantly (up to 30 hours for trimethoprim).
The terminal elimination half-life for rifaximin after oral administration ranges from 1.8 to 10 hours, with a mean of approximately 6 hours. The half-life is extended in hepatic impairment due to reduced clearance, and no dosage adjustment is recommended for renal impairment.
Both sulfamethoxazole and trimethoprim are primarily excreted via the kidneys. Sulfamethoxazole: ~30% as unchanged drug, ~50% as N4-acetyl metabolite; trimethoprim: ~80% as unchanged drug. Fecal elimination is minimal (<5%).
Rifaximin is primarily eliminated unchanged in feces via biliary excretion (approximately 97% of an oral dose). Renal excretion of unchanged drug accounts for <0.4% of the dose. Fecal elimination is the major route.
