SYMADINE vs TRIFLURIDINE

SYMADINE (amantadine) is a tricyclic amine that inhibits influenza A virus replication by blocking the viral M2 ion channel, which prevents uncoating of viral RNA. It also increases dopamine release and inhibits dopamine reuptake in the CNS, providing antiparkinsonian effects.
Trifluridine is a thymidine analog that inhibits thymidylate synthase and incorporates into DNA, leading to DNA damage and cell death.
100 mg orally every 12 hours; immediate-release formulation.
Topical: Apply one drop to affected eye every 2 hours while awake (maximum 9 drops/day) until re-epithelialization, then one drop every 4 hours for 7 days. Ophthalmic solution 1%.

The terminal elimination half-life is approximately 24 hours in patients with normal renal function. In patients with renal impairment (CrCl <50 mL/min), the half-life is significantly prolonged, requiring dose adjustment. The long half-life allows for once-daily dosing.
The terminal elimination half-life of trifluridine is approximately 12-18 hours. This prolonged half-life supports twice-daily dosing and provides sustained exposure for antiviral activity.
Renal elimination of unchanged drug accounts for approximately 90% of the administered dose. Biliary/fecal excretion is minimal (<5%).
Renal excretion accounts for approximately 40-50% of the administered dose, primarily as the inactive metabolite 5-trifluorothymidine. Fecal excretion is minimal (<5%). The remainder is eliminated via metabolic degradation.
