Clinical Essentials

TENUATE DOSPAN
BONTRIL PDM
Mechanism of Action
TENUATE DOSPAN

Releases norepinephrine from nerve terminals in the lateral hypothalamic feeding center, reducing appetite.

BONTRIL PDM

Phentermine is a sympathomimetic amine that acts as an appetite suppressant by stimulating the release of norepinephrine and dopamine in the hypothalamus, reducing food intake. Topiramate is a sulfamate-substituted monosaccharide that enhances GABAergic activity and inhibits glutamatergic neurotransmission via AMPA/kainate receptors, leading to appetite suppression and increased energy expenditure.

Indications
TENUATE DOSPAN

Short-term adjunct in exogenous obesity,FDA-approved for obesity management

BONTRIL PDM

FDA-approved: Chronic weight management (BMI ≥30 kg/m² or ≥27 kg/m² with at least one weight-related comorbidity) as an adjunct to a reduced-calorie diet and increased physical activity.,Off-label: None widely recognized.

Standard Dosing
TENUATE DOSPAN

25 mg orally three times a day, 1 hour before meals, or 75 mg extended-release orally once daily in the morning.

BONTRIL PDM

Oral: 5-10 mg once daily in the morning; maximum 20 mg/day. Oral disintegrating tablet: 5-10 mg once daily.

Pharmacokinetics

TENUATE DOSPAN
BONTRIL PDM
Half-Life
TENUATE DOSPAN

The terminal elimination half-life is approximately 4-6 hours in adults with normal renal function, though clinical effects may last longer due to tissue distribution.

BONTRIL PDM

Terminal elimination half-life is 12-15 hours in adults, prolonged to 20-30 hours in severe renal impairment (Cr Cl <30 m L/min).

Metabolism
TENUATE DOSPAN

Hepatic via CYP3A4 and other CYP enzymes

BONTRIL PDM

Phentermine: primarily renal excretion (unchanged). Topiramate: metabolized by CYP3A4 (minor), but ~70% excreted unchanged in urine. Also undergoes hydrolysis and glucuronidation.

Excretion
TENUATE DOSPAN

Renal excretion of unchanged drug and metabolites; approximately 85-90% of the dose is excreted in urine within 48 hours, with less than 5% in feces.

BONTRIL PDM

Renal: ~70% (unchanged), Fecal: ~30% (biliary excretion of metabolites).

Protein Binding
TENUATE DOSPAN

Approximately 20-30% bound to plasma proteins.

BONTRIL PDM

98% bound to albumin.

VD (L/kg)
TENUATE DOSPAN

Approximately 5-10 L/kg, indicating extensive tissue distribution.

BONTRIL PDM

0.25-0.35 L/kg, indicating distribution primarily in extracellular fluid.

Bioavailability
TENUATE DOSPAN

Rapidly absorbed from the gastrointestinal tract; absolute oral bioavailability is about 10-20% due to extensive first-pass hepatic metabolism.

BONTRIL PDM

Oral: 65-75% (first-pass metabolism); IM: 85-95%.
