Clinical Essentials

VISTARIL
ADVIL ALLERGY SINUS
Mechanism of Action
VISTARIL

Hydroxyzine is a piperazine derivative antihistamine that acts as a competitive antagonist of histamine H1 receptors, thereby suppressing histamine activity in the subcortical area of the central nervous system. It also has anxiolytic, sedative, antiemetic, and antispasmodic effects.

ADVIL ALLERGY SINUS

Pseudoephedrine is a sympathomimetic amine that directly stimulates alpha-adrenergic receptors, causing vasoconstriction of nasal mucosa and sinus vessels. Chlorpheniramine is an alkylamine antihistamine that competitively antagonizes histamine H1 receptors, reducing allergic symptoms. Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX-1 and COX-2) enzymes, decreasing prostaglandin synthesis and reducing pain, fever, and inflammation.

Indications
VISTARIL

Anxiety and tension associated with psychoneurosis,Pruritus due to allergic conditions (e.g., urticaria, atopic dermatitis),Sedation prior to dental or surgical procedures,Nausea and vomiting (off-label)

Special Populations

VISTARIL
ADVIL ALLERGY SINUS
Renal Adjustments
VISTARIL

No specific adjustment; use with caution in severe renal impairment due to potential accumulation of metabolites.

ADVIL ALLERGY SINUS

If GFR <30 m L/min: avoid use of ibuprofen component; pseudoephedrine dose interval may need to be increased (every 8-12 hours) due to reduced clearance.

Child-Pugh Class A: no adjustment; Child-Pugh Class B: reduce dose by 50%; Child-Pugh Class C: avoid use.

ADVIL ALLERGY SINUS

Child-Pugh Class A: no adjustment; Class B: use with caution, maximum ibuprofen dose 1200 mg/day; Class C: contraindicated due to risk of hepatotoxicity and renal impairment.

Pediatric Dosing
VISTARIL

Oral: 0.5-1 mg/kg every 4-6 hours; maximum 50 mg per dose (≤12 years); IM: 0.5-1 mg/kg every 4-6 hours.

ADVIL ALLERGY SINUS

Geriatric Dosing
VISTARIL

Start at lower end of dosing range (e.g., 25 mg oral 3-4 times daily); monitor for sedation and anticholinergic effects.

ADVIL ALLERGY SINUS

Initiate at lowest effective dose (1 tablet every 6-8 hours); monitor renal function and blood pressure due to increased risk of GI bleeding, cardiovascular events, and pseudoephedrine-induced hypertension.

FDA Black Box Warning

No FDA black box warning exists for this combination product. However, NSAIDs like ibuprofen carry a black box warning for increased risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal, especially with prolonged use or in patients with cardiovascular risk factors.

Warnings/Precautions
VISTARIL

May cause QT prolongation; use with caution in patients with risk factors (e.g., electrolyte imbalance, concomitant QT-prolonging drugs),Sedation and impaired cognitive/motor function; avoid driving or hazardous activities,Anticholinergic effects (e.g., urinary retention, constipation); use cautiously in elderly or patients with prostatic hypertrophy,Respiratory depression with concurrent CNS depressants,Use in pregnancy: avoid especially during early pregnancy; may increase risk of fetal abnormalities

ADVIL ALLERGY SINUS

Cardiovascular risk: NSAIDs increase risk of serious cardiovascular events. Gastrointestinal risk: NSAIDs can cause bleeding, ulceration, and perforation. Hypertension: Pseudoephedrine may elevate blood pressure. Avoid use with MAOIs or within 14 days of stopping. Caution in hyperthyroidism, diabetes, glaucoma, prostatic hypertrophy, and renal impairment.

Food Interactions
VISTARIL

Alcohol increases sedation and CNS depression; avoid concurrent use. No significant food interactions, but take with food if GI upset occurs.

ADVIL ALLERGY SINUS

Avoid alcohol due to increased risk of GI bleeding and liver toxicity. No known food interactions with chlorpheniramine or pseudoephedrine. Taking with food may reduce gastric irritation from ibuprofen.

Pregnancy & Lactation

VISTARIL
ADVIL ALLERGY SINUS
Teratogenic Risk
VISTARIL

First trimester: Limited human data; animal studies suggest no major teratogenic risk; fetal harm cannot be ruled out. Second and third trimesters: Hydroxyzine may cause neonatal withdrawal symptoms (irritability, tremors) with chronic maternal use near term; no evidence of structural anomalies.

ADVIL ALLERGY SINUS

First trimester: NSAIDs are associated with increased risk of miscarriage and congenital malformations (cardiac defects, gastroschisis). Third trimester: Risk of premature closure of ductus arteriosus, oligohydramnios, and necrotizing enterocolitis. Avoid use after 30 weeks gestation.

Lactation Summary
VISTARIL

Hydroxyzine is excreted in human milk; M/P ratio not established. Potential for adverse effects in infants (sedation, irritability). Use during breastfeeding only if clearly needed; monitor infant for drowsiness.

ADVIL ALLERGY SINUS

Ibuprofen and pseudoephedrine are excreted into breast milk. Ibuprofen has low milk/plasma ratio (0.01-0.07) and is generally considered compatible. Pseudoephedrine may reduce milk production and cause irritability in infants. Use with caution, especially in preterm infants.

Pregnancy Dosing
VISTARIL

No specific dose adjustments recommended in pregnancy; use lowest effective dose. Hydroxyzine may be less effective if used as an antiemetic due to altered clearance; consider alternative agents.

ADVIL ALLERGY SINUS

No specific dose adjustments recommended for pregnancy; however, use the lowest effective dose for the shortest duration. Avoid in third trimester. Pseudoephedrine dose remains standard; caution in hypertensive disorders.

Clinical Pearls
VISTARIL

VISTARIL (hydroxyzine pamoate) is a first-generation antihistamine with anxiolytic and sedative properties. It is often used for preoperative sedation, pruritus, and anxiety. Onset of sedation is rapid (15-30 minutes) but duration is short (4-6 hours). It has anticholinergic effects; caution in elderly and patients with glaucoma or prostatic hypertrophy. Do not administer intra-arterially or subcutaneously (risk of hemolysis or tissue necrosis). It is not a controlled substance, but has abuse potential. May cause significant somnolence; advise against driving or operating machinery.

ADVIL ALLERGY SINUS

Advil Allergy Sinus contains ibuprofen (NSAID), chlorpheniramine (first-generation antihistamine), and pseudoephedrine (decongestant). Avoid in patients with aspirin/NSAID allergy, severe hypertension, coronary artery disease, or MAOI use. Caution in elderly due to anticholinergic effects. Pseudoephedrine may cause insomnia and anxiety; avoid evening dosing.
