E-Z SCRUB
Clinical safety rating: caution
Comprehensive clinical and safety monograph for E-Z SCRUB (E-Z SCRUB).
E-Z SCRUB (chlorhexidine gluconate scrub) is a cationic bisbiguanide antiseptic with broad-spectrum antimicrobial activity. Its mechanism involves disrupting microbial cell membranes through binding to negatively charged cell wall components, leading to leakage of intracellular contents and cell death.
| Metabolism | Chlorhexidine is not significantly metabolized systemically; it is poorly absorbed through the skin. Absorbed amounts are primarily excreted unchanged in urine via glomerular filtration. |
| Excretion | Renal: >90% as unchanged drug within 24h; Biliary/fecal: <5% |
| Half-life | Terminal half-life 2-4 hours (prolonged in renal impairment, up to 12-24 hours in ESRD) |
| Protein binding | 99% bound to plasma proteins (mostly albumin) |
| Volume of Distribution | 0.4-0.6 L/kg (primarily confined to blood and interstitial fluid; minimal tissue penetration) |
| Bioavailability | Topical: negligible systemic absorption (<0.1%); not administered orally/IV |
| Onset of Action | Topical: 15-30 seconds for antimicrobial effect (scrub); oral/IV: not applicable (topical only) |
| Duration of Action | Single application: 2-6 hours of antimicrobial activity (slowly regenerating chlorhexidine on skin); surgical scrub: bacteriostatic effect for up to 6 hours |
Not applicable; E-Z SCRUB is a surgical scrub product containing chlorhexidine gluconate 4% and isopropyl alcohol 70%, for topical use only, not for systemic administration.
| Dosage form | SPONGE |
| Renal impairment | No adjustment required; topical application with negligible systemic absorption. |
| Liver impairment | No adjustment required; topical application with negligible systemic absorption. |
| Pediatric use | Not recommended for children under 2 months of age; for older children, use same as adults with caution. |
| Geriatric use | No specific adjustments; use as directed with caution due to potential skin sensitivity. |
| 1st trimester | Consult provider |
| 2nd trimester | Consult provider |
| 3rd trimester | Consult provider |
Clinical note
Comprehensive clinical and safety monograph for E-Z SCRUB (E-Z SCRUB).
| Breastfeeding | Chlorhexidine gluconate is not absorbed systemically after topical application, thus excretion into breast milk is negligible. The M/P ratio is not calculated due to minimal systemic absorption. Compatible with breastfeeding. |
| Teratogenic Risk | E-Z SCRUB is a topical antiseptic containing chlorhexidine gluconate and isopropyl alcohol. No fetal risks have been reported with topical application; systemic absorption is minimal. No animal reproduction studies have been conducted, and there are no adequate human studies in pregnant women. Use during pregnancy only if clearly needed. |

| Fetal Monitoring | No specific maternal or fetal monitoring is required for topical use. Observe for local skin reactions or hypersensitivity. |
| Fertility Effects | No data available on effects on fertility from topical use. Systemic absorption is minimal, and no reproductive toxicity is expected. |
