Use during pregnancy
1st trimester
Caution. Use for specific cardiac indications; labetalol preferred for hypertension.
2nd trimester
Caution. Monitor fetal growth.
3rd trimester
Caution. Monitor neonate for bradycardia and hypoglycemia.
Clinical note
Beta-1 selective blocker used for hypertension and maternal cardiac conditions (arrhythmias, peripartum cardiomyopathy, thyrotoxicosis). Less well-studied than labetalol for obstetric hypertension, but no significant signal for major malformations. Associated with FGR to a lesser degree than atenolol. Use labetalol preferentially for uncomplicated hypertension; reserve metoprolol for specific cardiac indications.
Breastfeeding
Safe. Compatible with breastfeeding; low milk transfer.
