OCUSULF-10
Clinical safety rating: caution
Comprehensive clinical and safety monograph for OCUSULF-10 (OCUSULF-10).
Sulfacetamide sodium is a sulfonamide antibiotic that inhibits bacterial dihydropteroate synthase, blocking folate synthesis and exerting bacteriostatic effects.
| Metabolism | Sulfacetamide is metabolized in the liver via acetylation and glucuronidation; excreted renally. |
| Excretion | Renal: ~70% as unchanged drug; biliary/fecal: ~30% as metabolites and unchanged drug. |
| Half-life | 1.5–2.5 hours (terminal) in adults; prolonged to 3–5 hours in renal impairment (CrCl <30 mL/min). |
| Protein binding | ~95% bound to albumin. |
| Volume of Distribution | 0.2–0.3 L/kg, indicating limited extravascular distribution. |
| Bioavailability | Oral: ~60–70% (first-pass metabolism ~30%). |
| Onset of Action | Oral: 30–60 minutes; IV: 5–10 minutes. |
| Duration of Action | 8–12 hours (dose-dependent); extended to 12–16 hours in renal impairment. |
| Molecular Weight | 514.5 |
Instill 1 drop into the affected eye(s) every 4 hours while awake.
| Dosage form | SOLUTION/DROPS |
| Renal impairment | No dose adjustment required for renal impairment. |
| Liver impairment | No dose adjustment required for hepatic impairment. |
| Pediatric use | Safety and efficacy in pediatric patients have not been established. |
| Geriatric use | No specific dose adjustment required in elderly patients. |
| 1st trimester | Avoid unless clearly needed; animal studies have shown teratogenic effects at high doses, but no adequate human studies. |
| 2nd trimester | Use only if potential benefit justifies potential risk to fetus; limited human data. |
| 3rd trimester | Use only if clearly needed; may cause fetal harm based on animal data. |
Clinical note

| Placental transfer | Crosses placenta in animals; human data limited but presumed. |
| Breastfeeding | Not known if excreted in human milk; caution due to potential for serious adverse reactions in nursing infants. Consider discontinuing nursing or drug. |

| Serious Effects |
Hypersensitivity to sulfacetamide or any componentSulfonamide allergy
| Precautions | Hypersensitivity reactions including Stevens-Johnson syndrome; use caution in patients with sulfonamide allergy; prolonged use may lead to overgrowth of nonsusceptible organisms including fungi. |
| Food/Dietary | No specific food interactions reported for ophthalmic sulfacetamide. As systemic absorption is minimal, dietary restrictions are not required. |
| Clinical Pearls | OCUSULF-10 (sulfacetamide sodium ophthalmic solution 10%) is a topical sulfonamide antibiotic used for ocular infections. Due to sulfonamide cross-sensitivity, avoid in patients with known sulfa allergy. Prolonged use may lead to overgrowth of nonsusceptible organisms including fungi. Solutions containing benzalkonium chloride as preservative may be absorbed by soft contact lenses; remove lenses before instillation. Transient stinging or burning upon application is common. In cases of severe infection, consider systemic antibiotic adjunct therapy. |

| L4 (Possibly Hazardous) |
| Teratogenic Risk | No adequate studies in pregnant women. In animal reproduction studies, no evidence of fetal harm at doses up to 4 times the maximum recommended human dose. Risk cannot be ruled out; use only if clearly needed. |
| Fetal Monitoring | Monitor intraocular pressure; no specific fetal monitoring required. |
| Fertility Effects | No studies on fertility; reproductive toxicity seen in animal studies at high doses. |
| Patient Advice | Wash hands before and after instillation to prevent contamination. · Do not touch the dropper tip to any surface, including the eye. · Remove contact lenses before use and wait at least 15 minutes after instillation before reinserting. · Apply prescribed number of drops only; do not share medication with others. · Notify your doctor if symptoms worsen or do not improve within a few days. · May cause temporary blurred vision; avoid driving until vision clears. · Store at room temperature, away from moisture and heat. · Complete full course even if symptoms resolve to prevent recurrence. · Report any signs of allergic reaction: rash, itching, swelling, difficulty breathing. |
