Clinical Significance

Assessing Liver Reserve

The Child-Pugh score provides a quick, bedside assessment of a cirrhotic patient's overall functional status. It was the primary tool for liver transplant allocation for decades before being largely superseded by the MELD score.

Beyond Transplant

Child-Pugh remains vital for adjusting drug dosages in hepatic impairment, predicting perioperative mortality in non-transplant surgery, and communicating the severity of liver disease between clinicians.

Functional vs. Biochemical

Unlike MELD, which is purely lab-based, Child-Pugh incorporates clinical markers like ascites and encephalopathy, providing a more "holistic" view of the patient's decompensation.

How it Works

The 5 Clinical Variables

Bilirubin: Total bilirubin level.
Albumin: Marker of synthetic function.
INR: Marker of coagulation.
Ascites: Absent, Mild, or Moderate/Severe.
Encephalopathy: Grade 1-4.

The Child-Pugh Classes

Class A (5–6 pts)Well-compensated; 1-yr survival 100%

Class C (10–15 pts)Decompensated; 1-yr survival 45%

Scoring Thresholds

A score of 10 or more (Class C) signifies severe liver failure and carries a 2-year survival probability of only 35%.

Clinical Pearls

Subjective Scoring Bias

The assessment of "Mild" vs. "Moderate" ascites is subjective. Use ultrasound to confirm if the clinical exam is indeterminate. Diuretic therapy can "improve" the score without changing the underlying liver function.

Albumin "Padding"

Exogenous albumin administration (e.g., after paracentesis) can temporarily raise the serum albumin level, potentially leading to an underestimate of the patient's true synthetic failure.

Surgical Risk Assessment

Class C patients are generally considered too high-risk for elective non-transplant surgery. Class A patients usually tolerate surgery well with a perioperative mortality < 10%.

Next Steps

Class B/C Management Bundle

Evaluate for Liver Transplant referral.
Initiate HCC screening every 6 months (US + AFP).
Perform screening endoscopy for esophageal varices.
Optimize diuretics and lactulose to maintain compensation.

Acute Decompensation Check

If a patient moves from Class A to Class B/C, perform a diagnostic paracentesis and screen for portal vein thrombosis (PVT).

Origins & History

University of Michigan (1964)

First proposed by surgeons C.G. Child and J.G. Turcotte to predict mortality in patients undergoing portosystemic shunt surgery for bleeding varices.

The King's College Refinement

Richard Pugh and colleagues at King's College Hospital, London, refined the score in 1973 by making the variables more objective. This "Modified Child-Pugh" score has remained a cornerstone of hepatology for over 50 years.
