| Breastfeeding | Acetaminophen is excreted into breast milk in low amounts (M/P ratio ~0.91-1.42) and is considered compatible with breastfeeding. Codeine is also excreted in breast milk; risk of infant opioid toxicity depends on maternal CYP2D6 phenotype. Ultra-rapid metabolizers may produce higher morphine levels. Use with caution, avoid in known CYP2D6 ultra-rapid metabolizers, and monitor infant for sedation and respiratory depression. |
