| Breastfeeding | Penicillin VK is excreted into human milk in low concentrations. The milk-to-plasma ratio is approximately 0.1. It is generally considered compatible with breastfeeding; however, monitor for potential effects on infant gastrointestinal flora (e.g., diarrhea, candidiasis). |

It has poor activity against beta-lactamase-producing bacteria. Take on an empty stomach for optimal absorption (1 hour before or 2 hours after meals). Renal impairment may require dose adjustment. Hypersensitivity reactions, including anaphylaxis, can occur; always ask about prior penicillin allergy. In pediatric patients, use weight-based dosing; suspension must be shaken well before each dose. Monitor for diarrhea as a sign of C. difficile infection. |
