Mechanism of Action
ACUVUE THERAVISION WITH KETOTIFEN

Ketotifen is a selective histamine H1-receptor antagonist and mast cell stabilizer that inhibits the release of inflammatory mediators such as histamine and leukotrienes from mast cells.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin binds to antithrombin III (ATIII), accelerating its inhibition of coagulation factors Xa and IIa (thrombin). Its anti-factor Xa to anti-factor IIa activity ratio is approximately 3.6:1.

Standard Dosing
ACUVUE THERAVISION WITH KETOTIFEN

One drop in each affected eye twice daily (approximately 8 hours apart) as needed. The lens should be removed prior to instillation and can be reinserted after at least 10 minutes.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

1 mg/kg subcutaneously every 12 hours or 1.5 mg/kg subcutaneously once daily. For prophylaxis: 40 mg subcutaneously once daily or 30 mg subcutaneously every 12 hours.

Spinal/epidural hematomas may occur in patients receiving enoxaparin who are undergoing neuraxial anesthesia or spinal puncture, resulting in long-term or permanent paralysis. Risk is increased by use of indwelling epidural catheters, concomitant use of other anticoagulants, or history of spinal surgery/deformity. Monitor for signs of neurological impairment and manage emergently.

Ketotifen ophthalmic solution has minimal systemic absorption (approximately 0.1% of administered dose). No adequate well-controlled studies in pregnant women. Animal studies showed no teratogenicity at doses up to 50 mg/kg/day orally. Risk to fetus is considered low when used topically as directed.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin does not cross the placenta and is considered low risk for teratogenicity. No increased risk of congenital anomalies has been reported in humans. ACUVUE THERAVISION WITH KETOTIFEN

Ketotifen is a mast cell stabilizer and antihistamine; contact lens must be removed before instillation and may be reinserted after 10 minutes. Do not use while wearing contact lenses. Advise patient to wait at least 5 minutes between different eye drops. The preservative benzalkonium chloride may be absorbed by soft contact lenses.

ENOXAPARIN SODIUM (PRESERVATIVE FREE)

Enoxaparin is a low molecular weight heparin (LMWH) preferred over unfractionated heparin for VTE prophylaxis due to predictable pharmacokinetics and no need for routine a PTT monitoring. Adjust dose for renal impairment (Cr Cl <30 m L/min). Protamine sulfate partially reverses (about 60%) its anticoagulant effect. Monitor for signs of bleeding, especially in elderly, low body weight (<45 kg), or those on antiplatelet agents. Avoid intramuscular injections. Spinal/epidural hematoma risk with neuraxial anesthesia; remove catheter at least 12 hours after last dose (24 hours if therapeutic dose).
