Mechanism of Action
ALPHACAINE HYDROCHLORIDE

Local anesthetic that reversibly blocks sodium ion channels in neuronal membranes, preventing the generation and propagation of action potentials.

TROMETHAMINE

Tromethamine is a proton acceptor that buffers hydrogen ions, correcting metabolic acidosis by increasing bicarbonate and base excess. It acts as a weak base with high buffering capacity.

Standard Dosing

Alphacaine Hydrochloride is an amide-type local anesthetic similar to lidocaine. Onset of action is 2-5 minutes with duration of 30-120 minutes depending on concentration and use of epinephrine. It is hepatically metabolized (CYP3A4) and renally excreted. Dose adjustment required in hepatic impairment. Risk of methemoglobinemia, especially in infants and patients on sulfonamides. Do not exceed maximum doses: 4.5 mg/kg plain, 7 mg/kg with epinephrine.

TROMETHAMINE

Tromethamine (THAM) is an amino alcohol that acts as a proton acceptor, used to correct metabolic acidosis when sodium bicarbonate is contraindicated (e.g., hypernatremia, hypercapnia). It is preferred in patients with lactic acidosis or respiratory acidosis because it does not generate CO2. Monitor serum potassium closely as it can cause hypokalemia.
