As a Ig G1 monoclonal antibody, it is expected to cross the placenta increasingly after the first trimester. The risk is likely low but cannot be excluded. Use only if clearly needed.

Pregnancy Category D. First trimester: High risk of congenital malformations including craniofacial, skeletal, CNS, and cardiac defects. Second and third trimesters: Risk of fetal growth restriction, prematurity, and neonatal myelosuppression.

Lactation Summary

It is not known whether obiltoxaximab is excreted in human milk. Monoclonal antibodies are typically excreted in breast milk at low levels with limited oral bioavailability due to gastrointestinal degradation. The M/P ratio is unknown.
