Fondaparinux is a synthetic pentasaccharide that selectively binds to antithrombin III, potentiating its inhibition of factor Xa. This prevents thrombin generation and clot formation.

| Breastfeeding | Fondaparinux is excreted into human breast milk in very small amounts. The milk-to-plasma (M/P) ratio has not been established in humans, but based on its high protein binding and large molecular weight, it is unlikely to accumulate in milk. No adverse effects have been observed in breastfed infants. Caution is advised due to limited data. |
| Teratogenic Risk | Fondaparinux is a synthetic pentasaccharide that does not cross the placenta due to its high molecular weight and low lipid solubility. No teratogenic effects have been reported in animal studies. In human pregnancies, limited data do not indicate an increased risk of major birth defects. Epidural or spinal hematomas may occur in patients anticoagulated with fondaparinux who receive neuraxial anesthesia or undergoing spinal puncture. These hematomas can result in long-term or permanent paralysis. Consider the benefits and risks before neuraxial intervention in patients anticoagulated or to be anticoagulated.
