Bendamustine is a bifunctional mechlorethamine derivative that forms electrophilic alkyl groups which covalently bond to DNA bases, resulting in interstrand DNA crosslinks, DNA single- and double-strand breaks, and ultimately apoptosis. It also inhibits several mitotic checkpoints and induces both apoptosis and necrosis in cancer cells.

Black Box Warning: TREANDA causes myelosuppression, including severe neutropenia and thrombocytopenia, which can lead to serious or fatal infections and bleeding. It also has a risk of infusion-related reactions and may cause severe skin reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis. Use in pregnant women can cause fetal harm.

| Clinical Pearls | TREANDA (bendamustine) is an alkylating agent with unique mechanisms including both alkylator and antimetabolite properties. It must be reconstituted and further diluted in NS or D5W; incompatible with D5/0.45% NS. Infusion reactions are common; premedicate with antihistamines, antipyretics, and corticosteroids. Extravasation risk is low but monitor site; no known antidote. Administer with caution in patients with renal or hepatic impairment; no dose adjustment needed for mild-moderate renal impairment but avoid in CrCl <30 mL/min. Monitor for myelosuppression, infections, and tumor lysis syndrome, especially in high tumor burden. Cases of severe skin reactions including Stevens-Johnson syndrome and toxic epidermal necrolysis have been reported; discontinue if severe reaction occurs. Also associated with progressive multifocal leukoencephalopathy (PML) and secondary malignancies. |
| Patient Advice | Take this medication exactly as prescribed by your doctor; it is given as an intravenous infusion over 30-60 minutes. · You will receive premedication before each dose to reduce the risk of allergic reactions; report any hives, itching, swelling, or breathing difficulty immediately. · Common side effects include nausea, vomiting, diarrhea, fever, fatigue, and low blood counts; report any signs of infection (fever, chills) or unusual bruising/bleeding. · Avoid alcohol and limit intake of foods or supplements that may affect liver function; no specific dietary restrictions but stay hydrated. · Drink plenty of fluids to help prevent tumor lysis syndrome; report any muscle cramps, nausea, vomiting, or decreased urination. · Do not receive any vaccines without your doctor's approval during treatment and for up to 6 months after. · This medication may cause fetal harm; use effective contraception during treatment and for 3 months after completion for females and for 6 months after for males. · Seek immediate medical attention if you develop rash, blistering, or peeling skin, mouth sores, or any new neurological symptoms such as confusion or vision changes. |
