Clinical management of Acute Pyelonephritis in Uganda

Outpatient (Mild/Moderate)

Ciprofloxacin 500 mg every 12 hours for 10-14 days. Paracetamol 1g every 6-8h for pain/fever.

Severe / Inpatient

Ceftriaxone 1 g IV once a day. After initial response, switch to oral Ciprofloxacin 750mg every 12h or Cefixime 200mg every 12h to complete 10 days.

Alternative Severe Regimen

Gentamicin 5-7 mg/kg IV in divided doses with or without Ampicillin 2 g IV every 6 hours.
