General Adult

Emergency Management

1.

Acute Transfusion Reaction (Mild)

Stop transfusion temporarily. Give Promethazine 25-50 mg IM/IV. Restart slowly if stable.

Treatment

1.

Indications

Symptomatic anaemia with Hb <7 g/dL. Hb <8 g/dL if cardiac disease or CNS symptoms present.

2.

Dose

Transfuse 1 unit at a time, then re-assess.


Pediatric

Treatment

1.

Indications

Hb ≤4 g/dL regardless of clinical state. Hb 4-6 g/dL if life-threatening complications (hypoxia, cardiac decompensation, acidosis).

2.

Dose

Whole Blood 20 ml/kg OR Packed Cells 10-15 ml/kg.


Pregnant Women

Treatment

1.

Pregnancy <36 weeks

Hb ≤5 g/dL regardless of state. Hb 5-7 g/dL if heart failure, pneumonia, or serious infection.

2.

Pregnancy >36 weeks

Hb ≤6 g/dL. Hb 6-8 g/dL if complications present.
