General Pediatric
Diagnosis
1.
Clinical Features
Primarily in children <3 years, usually viral. Early: Barking cough, hoarse voice, inspiratory stridor. Late: Severe dyspnoea, cyanosis, asphyxia.
Treatment
1.
Mild Croup
Isolate, rest, oral fluids, analgesics. Single dose steroid: Prednisolone 1-2 mg/kg OR Dexamethasone 0.15 mg/kg.
2.
Severe Croup
Admit. Humidified oxygen 30-40%. IV fluids. Steroids: Hydrocortisone slow IV (Child <1 yr: 25mg; 1-5 yrs: 50mg; 6-12 yrs: 100mg) OR dexamethasone 300 mcg/kg IM. If not controlled: nebulise adrenaline 0.4 mg/kg (max 5 mg).
3.
Life Threatening
Nasotracheal intubation or tracheostomy. Suspect bacterial (epiglottitis) if child appears critically ill.
