Evidence-based guideline: Antiepileptic drug selection for people with HIV/AIDS

Monitor for new-onset seizures, which occur in 2.6%–6.1% of patients (Class III) and may be caused by CNS opportunistic infections in up to 11% of cases.

Screen for peripheral neuropathy symptoms, which occur in 6.7%–52.5% of untreated HIV patients and develop in 17%–55% of patients after starting ARVs.

Screen for new-onset seizures, which occur in 2.6%–6.1% of patients (Class III) and are associated with a reported seizure incidence as high as 11% overall.

Dosage adjustment for efavirenz is generally not required, as valproic acid has no significant effect on efavirenz exposure (Level C).

A dosage increase of lamotrigine by ~50% may be required, as ritonavir/atazanavir reduces lamotrigine exposure by ~32% and half-life by ~27% (Level C).

Lamotrigine dosage adjustment is generally not required (Level C).

Midazolam dosage adjustment is generally not required, as raltegravir does not significantly affect CYP3A4 activity (Level C).

Perform frequent pharmacokinetic monitoring to ensure efficacy of the ARV regimen and prevent virologic failure (Level C).

Counsel patients that it is currently unclear whether dosage adjustment is necessary due to insufficient evidence (Level U).

Avoid Enzyme-Inducing AEDs (EI-AEDs) such as phenytoin, phenobarbital, and carbamazepine if the patient is on a Protease Inhibitor (PI) or NNRTI-based regimen (Level C).

Monitor for virologic failure (Viral Load > 400 copies/mL), as EI-AEDs increase failure rates from 27% to 63% (Odds Ratio 4.58; p=0.009).

Increase Lopinavir/Ritonavir dosage by ~50% because phenytoin reduces lopinavir AUC by 33% and ritonavir AUC by 28%. Phenytoin exposure is also reduced by 31%.

Reduce Zidovudine dosage as Valproic Acid increases Zidovudine exposure (AUC) from 0.65 to 1.17 mg/h/L.

Monitor for increased Lopinavir levels, which increase by 38% on average when Valproic Acid (500mg/day) is added.

Increase Lamotrigine dosage by ~50% because the combination reduces Lamotrigine AUC by 32% and half-life by 27%.

Double the Lamotrigine dose; Lopinavir/Ritonavir (400/100mg BID) reduces Lamotrigine exposure (AUC) by 50%.

Monitor for loss of ARV efficacy; Carbamazepine reduces Nevirapine half-life from 52 to 33 hours; Phenytoin reduces it from 46 to 27 hours.

Valproic Acid/Efavirenz, Raltegravir/Lamotrigine, and Raltegravir/Midazolam do not generally require dosage adjustments.

Exercise extreme caution when adding AEDs to enzyme-inducing TB medications (e.g., Rifampin), as this further increases metabolic complexity and interaction risk.

Utilize pharmacokinetic monitoring for both ARVs and AEDs if multiple interacting agents are required, to ensure virologic suppression and seizure control.
