General Adult
Diagnosis
Clinical Features
Focal neurological deficits as one-sided weakness (face, arm, leg). Difficulty in speaking/swallowing. Severe headache (especially in haemorrhage). Alteration of consciousness, convulsions.
Investigations
CT scan of the brain. Distinguish subtypes based on clinical course if neuroimaging is absent (e.g., abrupt severe headache for subarachnoid haemorrhage).
Treatment
General Care
Ensure airways. NPO until swallow assessed. IV or NGT for hydration. Control blood sugar with insulin if diabetic.
Ischaemic Stroke (Acute)
Aspirin 150-300 mg every 24 hours. Treat hypertension only if extreme (>220/120) or complications. DVT prophylaxis with enoxaparin 40 mg SC daily.
Ischaemic Stroke (Chronic)
Early mobilization and physiotherapy. Aspirin 75-100 mg once daily for life. Atorvastatin 40 mg daily for life.
Haemorrhagic Stroke
Supportive care. Refer for CT scan and neurosurgical evaluation.
