High-Risk / Multimorbid
Treatment
For patients of all ages with diabetes and ASCVD
Add high-intensity statin therapy to lifestyle therapy.
Goal for secondary prevention
Achieve LDL reduction of >=50% from baseline and a target goal of <55 mg/dL.
If goals not achieved on max tolerated statin
Addition of ezetimibe or a PCSK9 inhibitor with proven benefit is recommended.
General Adult
Treatment
For patients aged 40-75 years without ASCVD
Use moderate-intensity statin therapy in addition to lifestyle therapy.
For patients aged 40-75 years at higher CV risk (>=1 additional risk factor)
High-intensity statin therapy is recommended to reduce LDL by >=50% and reach goal <70 mg/dL.
Geriatric
Treatment
For adults >75 years already on statin
It is reasonable to continue statin treatment.
For adults >75 years initiating statin
Consider initiating moderate-intensity statin after risk/benefit discussion.
