Reticulocyte Production Index (RPI): Corrects for both the degree of anemia and the "shift" maturation time of young cells. Better indicator of marrow function than raw retic %.

When to Use
Clinical Use
Differentiating between causes of anemia (production vs. destruction).
Evaluating the bone marrow response to anemia.
Useful in normocytic anemia workup.
Section 2
Formula & Logic
Problem with Raw Reticulocyte %
The raw reticulocyte percentage can be misleading in anemia for two reasons: 1) The percentage rises simply because there are fewer total RBCs. 2) In severe anemia, reticulocytes are released early from the marrow (shift macrocytosis) and take longer to mature in the blood.
