Transfusion Safety (TACO): Circulatory overload is common and dangerous. Identification of risk factors PRIOR to transfusion allows for the use of pre-emptive diuretics and slow rates.
Pre-Transfusion Screen
No Risk Factors Selected
| Condition | Clinical Impact |
|---|---|
| Age > 70 years | Decreased cardiac reserve. |
| Known Heart Failure | Strongest independent predictor (LVEF < 45%). |
| Chronic Renal Failure | Impaired volume regulation and excretion. |
| Positive Fluid Balance | Pre-existing volume expansion before transfusion initiation. |
| Emergency Transfusion | Rapid infusion rates increase hydrostatic pressure. |
| Feature | TACO | TRALI |
|---|---|---|
| BP | Increased / Normal | Decreased / Normal |
| BNP | Significantly Elevated | Normal / Low |
| Response to Diuretics | Significance Improvement | No Response |
| Fluid Balance | Positive | Not necessarily positive |
