General Adult
Diagnosis
1.
Hypertensive Emergency
BP >180/110 mmHg WITH target organ damage (encephalopathy, acute angina/AMI, pulmonary oedema, acute kidney failure, acute aortic dissection, eclampsia).
2.
Hypertensive Urgency
BP >180/110 mmHg WITHOUT target organ damage.
Emergency Management
1.
Emergency Treatment
Admit. Give parenteral medicines. Aim to lower BP over 24 hours (not too rapidly). IV furosemide 40-80 mg. If aggressive lowering needed, use IV hydralazine 5-10 mg slowly over 20 mins.
2.
Urgency Treatment
Admit. Combination oral therapy (ACEI/ARB + CCB ± diuretics). Aim to lower BP over 48-72 hours.
