Calculation Steps

Assess area of involvement for each region, graded 0-6 (0=Clear, 1=1-9%, 2=10-29%, 3=30-49%, 4=50-69%, 5=70-89%, 6=90-100%).

Score each of the 4 severity signs from 0 (none) to 3 (severe) for each region.

Region Score = (Sum of 4 severity signs) × Area Score × Region Multiplier.

Total Score = Sum of the 4 Region Scores (Range: 0 to 72).

Clinical Pearls
Unlike SCORAD, EASI relies entirely on objective physician assessment. It does not factor in patient-reported symptoms like pruritus or sleep loss.
EASI is highly responsive to changes in severely extended disease, making it the preferred metric for moderate-to-severe AD trials.
Evaluate the average intensity of the signs across the entire involved area within a specific region, rather than just grading the worst lesion.
EASI-50, EASI-75, and EASI-90 (representing 50%, 75%, and 90% reductions from baseline scores) are the standard benchmarks for therapeutic efficacy in modern clinical trials.

The Eczema Area and Severity Index (EASI) was developed by Dr. Jon M. Hanifin and colleagues in the EASI Evaluator Group to provide a standardized, objective, and reproducible measure of atopic dermatitis severity for clinical trials, overcoming the subjective limitations of earlier tools.
