Input the GCS, Age, and scan findings to determine the ICH score and associated 30-day mortality risk.

When to Use
Predicting risk of symptomatic ICH after IV thrombolysis for stroke.
Identifying high-risk patients who require intensive neurological monitoring.
Patient and family counselling regarding thrombolysis complications.

Scoring Components
Variable
Scoring
NIHSS score
7–12 (1pt), 13–20 (2pts), ≥ 21 (3pts)
Age
≥ 75 years (1pt)
Serum Glucose
> 10 mmol/L or > 180 mg/dL (1pt)
Section 3
Pearls/Pitfalls
Glucose & Reperfusion
Hyperglycaemia is a major modifier of the blood-brain barrier. When reperfusion occurs via tPA, damaged capillaries are more susceptible to rupture. Maintaining normoglycaemia peri-thrombolysis is a key clinical priority.
