Formula

SPADI Total = (Pain Score x 50%) + (Disability Score x 50%) Pain subscale: 5 items, each 0-10 VAS Disability subscale: 8 items, each 0-10 VAS Total ranges 0-100 (0 = best, 100 = worst)

SPADI was developed at the University of Illinois in 1991 as a self-administered questionnaire for shoulder pain and disability.
