Pearls/Pitfalls
The Treatment Pivot
Most aggressive chemotherapy protocols and clinical trials require an ECOG score of 0 or 1. Patients with ECOG 2 may be considered for dose-reduced therapy, while ECOG 3-4 often mandates supportive care only.
ECOG vs. Karnofsky
While Karnofsky (0-100) provides more granularity, ECOG (0-5) is the global standard for its simplicity and superior inter-observer agreement among clinicians.
