When to Use

Overview

The Pain Assessment in Advanced Dementia (PAINAD) scale is a validated observational tool for estimating pain intensity in patients with severe cognitive impairment who have lost meaningful verbal communication. It evaluates five behavioral domains through a brief structured bedside observation, typically over 2–5 minutes.

Target Populations

Adults with moderate-to-severe Alzheimer's disease (FAST Stages 6–7)
Patients with other advanced dementias: vascular, Lewy body, frontotemporal dementia
Post-acute care and nursing home residents with cognitive impairment who cannot self-report pain
Pre- and post-procedural assessment in non-verbal adults (e.g., wound care, repositioning)
End-of-life palliative assessment in patients who have lost verbal capacity

Important Caveat

PAINAD does not diagnose pain — it quantifies pain-related behavioral indicators. Behavioral changes may also represent delirium, depression, anxiety, or discomfort unrelated to nociception. Always interpret PAINAD scores in context, and attempt to identify and address an underlying cause.

Depending on the patient's presentation and specific diagnostic requirements, you may also need to utilize the CPOT, Nu Desc Nursing Delirium Screening Scale, Fast Scale Functional Assessment Staging or the Akps Performance Status to formulate a comprehensive care plan.
