When to Use
When to Perform VCUG and Grade VUR
First febrile urinary tract infection (UTI) in children under 2 years (especially boys)
Recurrent febrile UTIs in older children
Prenatal hydronephrosis with persistent postnatal dilation
Abnormal bladder on ultrasound (thickened wall, diverticula, or ureterocele)
Family history of VUR or renal scarring in siblings
Neurogenic bladder or voiding dysfunction with recurrent infections
Follow-up after endoscopic injection or ureteral reimplantation to assess resolution
Pre-transplant evaluation in select cases
Clinical Importance
Vesicoureteral reflux (VUR) is the retrograde flow of urine from the bladder into the ureter and/or kidney. It is a major risk factor for pyelonephritis and renal scarring in children. The International Reflux Study Committee grading system, based on voiding cystourethrogram (VCUG), remains the gold standard for classifying severity and guiding management decisions between conservative observation, antibiotic prophylaxis, endoscopic therapy, and surgical reimplantation.
