Functional Activities Questionnaire (FAQ)

FAQ: Functional Activities Questionnaire. To be completed by a knowledgeable informant. Rate the patient\'s highest level of performance in the past month.

IADL Informant Report (0/10)

1. Writing checks, paying bills, balancing checkbook.

2. Assembling tax records, business affairs, or papers.

3. Shopping alone for clothes, household necessities, or groceries.

4. Playing a game of skill, working on a hobby.

5. Heating water, making a cup of coffee, turning off stove.

6. Preparing a balanced meal.

7. Keeping track of current events.

8. Paying attention to, understanding, or discussing a TV show, book, or magazine.

9. Remembering appointments, family occasions, holidays, medications.

10. Traveling out of neighborhood, driving, or arranging to take buses.

Scoring Logic

The FAQ is a 10-item instrument. Each item is rated 0 (Normal) to 3 (Dependent). Total score ranges from 0 to 30.

Interpretation

0–8 pointsNormal / Minimal Impairment
≥ 9 pointsClinically Significant Impairment
NoteA score of 9 is the standard threshold for identifying impaired function secondary to cognitive decline.
