Scoring Logic
The NPI evaluates 12 behavioral domains. For each domain present, the clinician rates: (1) Frequency (1-4), (2) Severity (1-3), and (3) Caregiver Distress (0-5). The Domain Score is Frequency × Severity (0-12).
