Guidelines & Evidence

Clinical Details

Section 1

When to Use

When to Use

Assessment of mania severity in patients with Bipolar I Disorder
Screening for mixed features (concomitant depressive symptoms) in manic episodes
Evaluating the presence and impact of psychotic symptoms in mania
Monitoring treatment response across the full spectrum of manic phenomenology
Section 2

Formula & Logic

Scoring Logic

The MSS consists of 14 clinician-rated items. It uniquely highlights the three core factors of mania: Elation/Activation, Psychosis/Hyper-activity, and Irritability/Dysphoria.

Interpreting Mixed Features

High scores on the Irritability/Dysphoria items in the presence of manic activation suggest a mixed-episode presentation with higher risk for suicide and treatment resistance.
Section 3

Evidence Appraisal

Validation

A mania symptom scale validated for mixed-state and psychotic mania.

Cassidy F et al. • Am J Psychiatry.. 1998;155(15):1741-7. Found the MSS to be superior to the YMRS for capturing mixed-state phenomenology.
