When to Use

When to Use

All patients admitted to acute hospital, ICU, long-term care, or home health settings.
Within 8 hours of admission and at each change of care level per NPUAP/EPUAP guidelines.
Serial monitoring in high-risk patients: elderly, neurologically impaired, malnourished, or incontinent.

Standard of Care

The Braden Scale is mandated by the Joint Commission and CMS as the reference pressure injury risk tool in US acute care hospitals. It is validated in acute care, ICU, long-term care, and home health populations.
