Berk M et al. • J Eval Clin Pract.. 2008;14(6):979-983. PMID: 18462279. 786 consecutive admissions over 24 months. CGI-S completion: 79.4% at admission, 78.1% at discharge; CGI-I completion: 77.6%. Admission/discharge CGI-S correlated at r = 0.40. Indirect improvement (CGI-S difference) correlated with direct CGI-I at r = 0.71. The CGI showed similar trends to HoNOS, MHQ-14, and DASS-21.

Unrelated medical/psychiatric adverse events in patient narratives significantly biased CGI-S and CGI-I ratings (P < .04). 87% of investigators believed medical AEs would not affect CGI-S ratings — but the data showed they did (P < .03).

CGI-I strongly correlated with concurrent CGI-S and CGI-S change from baseline. Social anxiety symptoms accounted for the largest fraction of CGI-S variance; impairment and depression added significant incremental variance.

Effect sizes moderate and comparable to established instruments. CGI-SCA 5 = PANSS 97-101; CGI-C-SCA 2 = 33-38% PANSS reduction. Raters distinguished positive from manic symptoms well (>90% agreement) but struggled with negative vs. depressive.
