Clinical Significance

Screening for Liver Scarring

NAFLD is extremely common, but only a minority of patients develop advanced fibrosis. Identifying those at low risk is essential for avoiding unnecessary biopsies and focusing resources on those who need it most.

The Power of Negative Prediction

The BARD score is most useful for its Negative Predictive Value (NPV). A low score effectively rules out advanced fibrosis, providing peace of mind for both the clinician and the patient.

Simplicity in Primary Care

Because it uses only BMI and standard blood tests, the BARD score is ideal for use in the primary care setting as a first-line screening tool.

How it Works

The 3 Scoring Factors

BMI ≥ 28 kg/m²: +1 Point.
AST/ALT Ratio ≥ 0.8: +2 Points.
Diabetes Mellitus: +1 Point.

Total Score Range

The score ranges from 0 to 4.

Interpretation Threshold

Score 0–1: High NPV for advanced fibrosis (F3/F4). Fibrosis is highly unlikely.

Clinical Pearls

Diabetes as a Primary Driver

Type 2 Diabetes is the single most important clinical risk factor for the progression of fatty liver to cirrhosis. Even without other factors, a diabetic patient requires close liver monitoring.

Low Specificity

While a low score rules out fibrosis, a high score (2–4) does NOT guarantee it. Roughly 50% of patients with a high BARD score will actually have low-stage disease.

The Platelet Tie-Breaker

If the BARD score is indeterminate, check the platelet count. Thrombocytopenia is a much more specific marker for advanced fibrosis and portal hypertension.

Next Steps

Low Risk (Score 0-1)

Manage metabolic syndrome components.
Counsel on weight loss and exercise.
Repeat BARD or FIB-4 screening every 2-3 years.

Indeterminate/High Risk (Score 2-4)

Order a second-tier non-invasive test (FibroScan).
Refer to Hepatology if stiffness > 8 kPa.
Optimize glycemic control to slow fibrosis progression.

Origins & History

The Army Liver Specialist

Developed by Dr. Stephen Harrison, a leading figure in NASH research and a former US Army physician. The BARD score was designed to be "field-ready" for clinicians without specialized equipment.

Evolution of NAFLD Nomenclature

The BARD score was developed when "NAFLD" was the standard term. Recently, the medical community has shifted toward "MASLD" (Metabolic-Associated Steatotic Liver Disease) to better reflect the underlying metabolic drivers used in this score.
