Clinical Significance

Diagnosis of ACLF

Acute-on-Chronic Liver Failure (ACLF) is not just worsening cirrhosis; it is a systemic inflammatory syndrome with organ failure. CLIF-SOFA provides the objective definition for when an organ system has officially "failed" in a cirrhotic patient.

A Liver-Specific ICU Tool

The standard SOFA score used in general ICUs is less accurate for cirrhotic patients. CLIF-SOFA incorporates INR and the West Haven encephalopathy scale, which are more relevant than platelets and GCS for this population.

Prognostic Power

By assessing six organ systems simultaneously, CLIF-SOFA accurately predicts short-term mortality, helping to guide decisions about ICU intensity and transplant urgency.

How it Works

The 6 Organ Systems (Scored 0–4)

Liver: Bilirubin thresholds.
Kidney: Creatinine or Dialysis (RRT).
Brain: West Haven Grade for HE.
Coagulation: INR or Platelet count.
Circulation: MAP and Vasopressor need.
Lungs: PaO2/FiO2 ratio.

ACLF Grading Logic

ACLF is graded by the number of organ failures (Scores of 3 or 4 in each system). ACLF Grade 1 (one organ failure), Grade 2 (two failures), Grade 3 (three or more failures).

Mortality Correlation

ACLF Grade 3 carries a 28-day mortality of approximately 75%.

Clinical Pearls

The Kidney Weighting

Renal failure is the most common and most lethal organ failure in ACLF. Any score of 3 or 4 in the kidney system mandates immediate, aggressive intervention.

RRT automatically = 4

Any patient on Renal Replacement Therapy (Dialysis/CRRT) for acute kidney injury is automatically scored 4 for the kidney system, regardless of their creatinine level.

Invasive Monitoring

If the circulatory or lung scores are ≥ 2, early placement of invasive arterial and central venous monitoring should be considered in the ICU.

Next Steps

ACLF Grade 2-3 Management

Urgent status evaluation for Liver Transplant.
Start broad-spectrum empiric antibiotics.
Optimize hemodynamics with Albumin and Vasopressors.
Daily monitoring for new organ failure (CLIF-SOFA trajectory).

Origins & History

EASL-CLIF Consortium

The score was developed by the European Foundation for the Study of Chronic Liver Failure, a network of top European hepatologists aiming to standardize ICU care for liver patients.

Modern ACLF Era

The introduction of CLIF-SOFA in 2013 marked the beginning of the "ACLF Era," where liver failure is viewed as a systemic inflammatory condition rather than just a local liver problem.
