Matched clinical terms: screening, screening — may affect guidance relevant to VOMS (Vestibular/Ocular-Motor Screening).
VOMS (Vestibular/Ocular-Motor Screening)
Pre-Test Baseline (0–10)
Assessment Stage 1 of 7
Smooth Pursuits
Track target moving H/V
VMS Warning
Perform visual motion sensitivity last. It is the most provocative task and may cause severe nausea.
Guidelines & Evidence
Clinical Details
Section 1
When to Use
When to Use
Screening for vestibular and oculomotor dysfunction following sport-related concussion.
Identifying sub-types of concussion (vestibular vs. ocular) to guide rehabilitation.
Initial assessment (baseline or post-injury) and monitoring treatment progress.
Clinical Utility
VOMS is highly sensitive in identifying concussed athletes early, even when cognitive or balance tests (e.g., mBESS) are normal.
Section 2
Formula & Logic
Assessment Domains
Smooth Pursuits
Tracking a slow-moving target
Saccades
Rapidly shifting gaze between two targets (H/V)
Near Point Converg. (NPC)
Distance where double vision occurs (Abnormal: ≥5cm)
VOR
Maintain gaze focus during head motion (H/V)
Visual Motion Sens. (VMS)
Focus on thumb during full body rotation
Symptom Rating
Before and after each task, the patient rates four symptoms on a 0–10 scale: Headache, Dizziness, Nausea, and Fogginess (HDNF).
Positive Thresholds
Increase in any HDNF symptom by ≥2 points from baseline.
NPC distance ≥5 cm (measured in 3 trials).
Section 3
Pearls/Pitfalls
Direct Comparison
Unlike the SCAT6 which focus on balance and cognition, VOMS specifically targets the integration of visual and vestibular inputs, which are often the primary drivers of persistent post-concussive symptoms.
Clinical Pearls
Always perform a baseline symptom check BEFORE starting the VOMS maneuvers.
If symptoms spike significantly during one task, pause until they return to baseline before proceeding.
Abnormal NPC (Convergence insufficiency) is a strong predictor of prolonged recovery.

Next Steps
Referral & Rehab
01
Vestibular Therapy: Indicated for positive VOR/VMS provocation.
02
Vision Therapy: Indicated for positive NPC or Saccade/Pursuit deficits.
03
Serial Tracking: Monitor VOMS scores weekly to judge the effectiveness of rehab.
