Evaluation of athletes with acute or chronic injuries requiring treatment with glucocorticoids, local anesthetics, or other prohibited substances.

Assessment of athletes with endocrine disorders (e.g., hypogonadism, growth hormone deficiency) requiring hormone replacement therapy.

Review of athletes with neurological or psychiatric conditions (e.g., epilepsy, depression, ADHD) on prohibited medications.

Post-doping control notification when an athlete has a legitimate medical need for a prohibited substance.

Surveillance of athletes with known TUEs to ensure ongoing compliance and medical necessity.

Key Indication - WADA International Standard for Therapeutic Use Exemptions (ISTUE)

The TUE Criteria Checklist is essential for determining whether an athlete qualifies for a TUE under the WADA ISTUE. The four key criteria are: (1) The prohibited substance or method is needed to treat a diagnosed medical condition supported by relevant clinical evidence; (2) The therapeutic use of the prohibited substance or method will not produce any additional enhancement of performance beyond what is expected by a return to normal health; (3) There is no reasonable therapeutic alternative to the use of the prohibited substance or method; and (4) The necessity for the use of the prohibited substance or method is not a consequence of the prior use (without a TUE) of a substance or method that was prohibited at the time of use.

When Not to Use / Contraindications

Athletes without a clearly diagnosed medical condition supported by objective medical evidence.

Use of a prohibited substance solely for performance enhancement without a therapeutic need.

Availability of a permitted therapeutic alternative that can effectively treat the condition.

Prior use of the prohibited substance without a TUE that led to the current medical condition (e.g., anabolic steroid-induced hypogonadism).

Conditions that are self-limiting or can be managed with non-prohibited treatments (e.g., mild asthma controlled with inhaled beta-2 agonists within permitted doses).

Athletes who are not subject to WADA rules (e.g., recreational athletes not in a testing pool).

Emergency or acute situations where immediate treatment is necessary and TUE application is retrospective (though a retroactive TUE may be considered in specific circumstances).
