When to Use

Pre-participation screening for athletes in collision/contact sports (e.g., rugby, American football, ice hockey) to identify those at risk for sudden cardiac death.

Evaluation of athletes presenting with exertional syncope, near-syncope, chest pain, palpitations, or unexplained dyspnea.

Follow-up assessment for athletes with known or suspected cardiac conditions (e.g., hypertrophic cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy).

Sideline assessment of athletes who collapse or experience cardiac symptoms during training or competition.

Serial monitoring of athletes with a family history of sudden cardiac death or inherited cardiac conditions.

Key Indication - Risk Stratification for Sudden Cardiac Death

The REDSCAT (Rapid ECG-based Decision Support for Cardiac Assessment in Athletes) tool is primarily indicated for risk stratification of sudden cardiac death (SCD) in athletes aged 12-35 years. It integrates ECG findings with clinical history and physical exam to generate a composite risk score. A score ≥5 warrants immediate cardiology referral and restriction from competitive sports pending further evaluation, as per the 2020 ESC guidelines and AHA/ACSM recommendations.

When Not to Use / Contraindications

Do not use as a standalone diagnostic tool for structural heart disease; it is a screening adjunct.

Avoid in athletes with known pacemakers or implantable cardioverter-defibrillators (ICDs) as ECG interpretation is confounded.

Not validated for athletes >35 years old; alternative tools (e.g., SCORE) are preferred.

Contraindicated in acute hemodynamic instability or ongoing cardiac arrest; follow ACLS protocols.

Do not use in athletes with electrolyte disturbances or on medications that affect ECG (e.g., antiarrhythmics) without correction.
